Below we describe the information in the fields in the provider directory. The doctor or hospital gives CCP this information on their application when they join the health plan. CCP checks this information at least every 3 years. Sometimes CCP does not receive a notice that the information has changed. If a doctor or hospital makes a change to any of the information below, CCP updates the provider directory within 7 days of learning of the change. Name: The doctor or hospital that is part of the CCP network.
Specialty: The area of medicine the doctor provides care to patients. An example is a doctor who works only with the heart, cancer, or bones.
Gender: A doctor’s sex, either Male or Female.
Phone Number: The doctor’s office or hospital phone number.
Location: The doctor’s office or hospital address.
Location Name: Name of the doctor's office or hospital address.
Hospital Accreditation: Hospitals gets accredited when they meet quality and safety standards. The hospital gets reviewed by national organizations to see if they meet these standards.
Hospital Quality: The hospital’s performance rating. The hospital reports the safety and quality of its care.
Accepting new patients: The doctor is taking new patients. Some doctors might have all the patients they can fit in, while still giving you the best care. For more information, please call the doctor’s office.
Accessible with a Wheelchair: The office is handicap accessible.
ASL Interpreter Available: The office can translate into American Sign Language.
Cultural Competency Training: The office staff is trained to meet the needs of people of all different cultures.
Hours: When the office or hospital is open to see patients.
Languages: The languages a doctor speaks.
Groups: The practice a doctor works in. A doctor may work with other doctors. They might also work in different offices or clinics.
Hospitals: The network hospitals a doctor can see their patients in.
Board Certifications: A doctor gets a certificate after completing extra training in their specialty. This certificate expires, so CCP checks it at least every 3 years. Website: The internet address that the doctor and practice information can be found.
Preferred Provider: A doctor that meets strict quality standards. The doctor provides an excellent patient experience.
Essential Provider: Essential Providers are certain providers that are statewide resources for all Managed Care Plans in all regions. CCP must include these essential providers in its network, even if the provider is located outside of the region served by CCP.
